Referrals

If you are a professional and need to complete a referral form for a person who needs supported accommodation, find guidance for the:

  • single homeless pathway referral
  • YMCA mother and baby referral.

Single homeless pathway referral

The single homeless pathway is for single people experiencing homelessness on the Island, and it offers a range of support and accommodation options to suit their needs.

If you're an approved professional working with a person and wish to refer them into this pathway:

Someone from the housing team will contact the person and discuss their situation to consider their most suitable options.

If you need any further guidance about the single homeless pathway or need help to complete the referral form, email Homelessintervention@iow.gov.uk.

  • pregnant (from 27 weeks onwards)
  • has children two years old or younger.

If you're an approved professional working with a mum or mum-to-be and wish to refer them into this service:
